Artificial intelligence is often promoted as a way to reduce healthcare costs.

But a new analysis from the Blue Cross Blue Shield Association raises a different possibility.

According to the association's analysis of claims data, increased use of AI coding tools coincided with a sharp rise in patients being classified as having complex medical conditions.

BCBSA estimates that the resulting increase in healthcare spending added approximately $942 million to costs for BCBS companies between 2023 and 2025.

The key issue is medical coding.

Hospitals use diagnosis codes to document patient conditions and determine reimbursement.

AI systems can help identify diagnoses that might otherwise be missed during the coding process.

That can improve documentation.

But BCBSA argues that some of the additional diagnoses were not accompanied by corresponding changes in treatment.

In other words, the coding changed while the care delivered did not.

That distinction is critical.

The BCBSA analysis is an insurer perspective, and the findings are part of a broader dispute between healthcare providers and insurers over coding, reimbursement and medical complexity.

It should therefore not be treated as proof that AI coding itself caused all of the additional spending.

But it does highlight a legitimate question.

What happens when AI becomes extremely good at finding billable complexity?

Healthcare organizations need to balance accurate documentation with appropriate reimbursement.

As AI becomes more deeply embedded in billing systems, auditing and transparency become increasingly important.

The technology may still produce significant efficiency gains.

But healthcare AI cannot be evaluated only by whether it saves administrative time.

It also needs to be measured against outcomes such as:

Did patient care improve? Did documentation become more accurate? And did the financial incentives remain aligned with actual care?

Those questions will become increasingly important as AI enters hospital revenue cycles.
